Please list the date you called your vets office here -----&gt;<br/><br/>11. Is all your pets in the house spayed &amp; neutered?<br/><br/>12. Has your dog been on heart worm preventative and where did you purchase it from?<br/><br/>13. This dog is not house broke. how are you going to house break the pet?<br/><br/>14. Have you ever had to rehome a pet? Or have you ever surrendered a pet at a humane society or shelter?<br/><br/>15. What interested you in this dog?<br/><br/>16. Where is this pet going to sleep or spend most of its time?<br/><br/>17. Please list the date you are available to drive here and potentially adopt. He is available anytime. <br/><br/>18.
