Has your dog been on heart worm preventative and where did you purchase it from?<br/><br/>13. This dog is not house broke. how are you going to house break the pet?<br/><br/>14. Have you ever had to rehome a pet? Or have you ever surrendered a pet at a humane society or shelter?<br/><br/>15. What interested you in this dog?<br/><br/>16.
