After submitting, scroll to the bottom to reach the questionnaire. Potential Adopter Questionnaire: Provides the information needed to contact you and move forward. Your application is not submitted until both forms are completed. Step 3: Application Review Applications are reviewed Tuesday–Saturday (12–5 pm).
