Once we have received your completed questionnaire someone will be in touch to set up an appointment for you to come in and meet the pet(s) you are interested in. If you have questions prior to completing the questionnaire please email info@clarkshelter.org or call (859)737-0053. Appointments will not be scheduled until the questionnaire is completed.
